PARTICIPATION AGREEMENT & LIABILITY RELEASE

Read before signing. Version MC-TC-2026-10-10-v1.

Program and released parties

This agreement applies to the registered athlete’s MAT CLASH wrestling practices, instruction, drills, live wrestling, conditioning, camps, clinics and related supervised program activities from the signing date for one year. “Released Parties” means MAT CLASH Training Center, MAT CLASH Wrestling Corp., MAT CLASH Team Gear, The On The Mat Foundation, and their program-related affiliated entities, owners, directors, officers, employees, coaches, volunteers, agents and representatives, together with participating facility owners and operators, only in connection with these program activities.

Acknowledgment and voluntary assumption of risks

Wrestling is a strenuous contact sport. Risks include falls, throws, collisions, contact with athletes, mats or equipment, cuts, bruises, sprains, fractures, dental injuries, concussion, head, neck or spinal injury, paralysis, communicable illness including skin infections, permanent disability and death. Injuries can occur during instruction, drilling, live wrestling, conditioning or use of facilities, including from another participant’s actions or ordinary negligence. I understand that protective equipment and supervision cannot eliminate all risks. I voluntarily accept the inherent risks for my own participation. As parent or legal guardian, I acknowledge these risks and authorize the minor athlete’s participation, subject to applicable law.

Adult release of claims — including ordinary negligence

TO THE FULLEST EXTENT PERMITTED BY TENNESSEE LAW, I, THE ADULT SIGNER, RELEASE AND DISCHARGE THE RELEASED PARTIES FROM MY OWN CLAIMS arising from participation in or attendance at the covered activities, including injury, illness, death or property damage CAUSED BY THE ORDINARY NEGLIGENCE OF A RELEASED PARTY, including ordinary negligence in instruction, supervision, premises or equipment. This release applies only to claims I legally have authority to release. It does not release gross negligence, recklessness, intentional misconduct, abuse, or any liability or legal duty that cannot lawfully be waived. It does not require a parent to indemnify a Released Party for a minor’s injury claim.

Minor participants and parental consent

If the athlete is under 18, I certify that I am their parent or legal guardian and have authority to authorize participation and emergency care. I sign in my individual capacity and as the athlete’s parent or legal guardian. I understand that Tennessee law limits a parent’s ability to release a minor’s claims, and this agreement does not extinguish a minor’s rights where prohibited by law. The adult release applies to my own claims only to the extent legally permitted. No promise of immunity from all claims is made.

USA Wrestling / AAU membership and verification

I certify that the membership information entered below is accurate and identifies the athlete’s current USA Wrestling and/or AAU athlete membership. The athlete must maintain the membership required for the specific practice or activity and present proof to the coaching staff before participating. A USA Wrestling card and an AAU card are not automatically interchangeable. Staff must verify that the membership, club affiliation and activity requirements are met. Membership does not guarantee insurance coverage or payment of a claim; coverage depends on applicable policies, exclusions, eligibility, sanctioning and other conditions. I will notify the program of expiration, lapse or changes and will not permit participation until required membership is verified.

Fitness, safety and emergency medical authorization

I will disclose participation restrictions and necessary safety information privately to the coaching staff before participation, follow instructions, use appropriate equipment, report injuries and symptoms promptly, and stop participation when instructed or when unsafe. The athlete will not participate with a suspected concussion or contagious condition until properly cleared under applicable requirements. I authorize staff to contact emergency services and arrange reasonable emergency medical care when needed, including when a guardian cannot be reached. This consent does not waive any nonwaivable rights or authorize unqualified staff to practice medicine. Responsibility for medical charges is determined by applicable law and insurance; the program does not promise to pay or insure every expense.

Agreement, electronic signature and records

I have read the entire agreement, understand that the adult release gives up certain legal rights, and sign voluntarily. I certify that I am at least 18 and that the information supplied is accurate. By typing my full legal name, checking the required acknowledgments and submitting, I intend to electronically sign this exact agreement and consent to its electronic transmission and retention with the registration information. I may save or print a copy before submission and request a copy from matclashwrestling@gmail.com. Registration does not itself approve participation; staff must confirm eligibility and membership. Tennessee law governs to the extent permitted. If a provision is unenforceable, the remaining lawful provisions remain effective. A new agreement is required after one year or a material change to the terms.

Your registration, signature, acknowledgments and full agreement are sent to MAT CLASH by email. See our privacy policy. Save a copy before submitting.

After submitting, contact our coaches to confirm the current schedule and next steps for your first practice. There is no checkout. Donations through the Foundation are welcome but entirely optional.
